Physical therapy can seem like a daunting undertaking, especially when you know little about what landed you in the clinic in the first place. Most healthcare providers don't do a great job at outlining your recovery timeline because it is based on the individual and so many variables play a part in the rate of healing. In this blog post, we'll cover what we do know about various types of tissue healing and what you can expect when you are consistent and compliant with your care!


Recovering from injuries involves a multilayered journey of tissue healing, each with its own unique timeline. Whether it's muscles, tendons, ligaments, or bones, understanding the stages of recovery is crucial. Let's delve into the timeline of tissue healing and explore how consistent physical therapy can significantly shorten recovery time.


 

TISSUE TYPES

Muscle Healing (Varies)

  • Has a faster healing time than tendons, ligaments and bones due to its vast blood supply

  • Physical therapy emphasizes

    • Early mobilization to prevent muscle atrophy and promote optimal healing.


Tendon and Ligament Healing (Weeks to Months)

  • These connective tissues heal more slowly due to lower blood supply.

  • Physical therapy emphasizes

    • Stimulating surrounding tissue through manual therapy, safe mobilization and low loaded exercises increases blood supply adjacent to the tendon or ligament, allowing for faster healing.

  • In a healthy individual, usually takes about 6 weeks

  • Physical therapy emphasizes

    • Weight-bearing exercises under proper guidance once appropriate plays a crucial role in bone remodeling.


 

GENERAL HEALING OUTLINE


Inflammatory Phase (Days 0-4)

  • In the initial days, the body responds to injury with inflammation.

  • Blood vessels widen, allowing immune cells to remove damaged cells and debris.

  • Physical therapy focuses on pain management and gentle movement to prevent stiffness.

  • Your PT can help you debunk myths that have become commonplace in this phase

    • i.e. Too much ice can actually prolong healing - some inflammation is good, and you cannot move onto the next phase without it!

Proliferative Phase (Days 4-21)

  • New tissue starts to form

  • Fibroblasts produce collagen, essential for tissue strength if it is laid down properly (see next bullet).

  • Controlled exercises in physical therapy aid in promoting proper tissue alignment and preventing scar tissue formation.


Remodeling Phase (Day 21-1 Year)

  • Collagen fibers align, and the tissue gains strength.

  • Physical therapy intensifies with a focus on functional exercises, gradually restoring range of motion and flexibility.

  • Supplemental modalities (ice, heat, e-stim, laser, pneumatic compression, etc.)
