Let's Get Started!

Here are a few things you can expect on your first visit to physical therapy!

Your first visit to Out of the Box is called your Initial Evaluation. This is a one-on-one session with the Doctor of Physical Therapy (DPT) spent to analyze your posture, flexibility, strength, mobility, movement patterns, and discuss your goals. You will get a taste of what a treatment session will look like, and your DPT will discuss your treatment plan with you. All of this will take about 1-hour, depending on your specific needs. Each follow-up visit will also be scheduled for 1-hour.

New Patient Forms

If you are a new patient and would like to ensure you spend as much time as possible with the Doctor of Physical Therapy during your first visit, please fill out our forms prior to coming to your Initial Evaluation! You will automatically receive an email to complete your forms online when you book your first appointment!

Please bring the follow to your first visit:

  • Proper forms of identification

  • Insurance cards (if you have coverage)

  • List of any medications you are currently taking

  • Prescription/Referral (not required - read below!)

  • Loose fitting clothing that easily exposes area of treatment

Above is just a copy for your records of the document the Agreement of Release & Liability Waiver is referring to. You do not need to print or fill out this form. 

Insurance & Payments

We accept commercial insurance for physical therapy and work with most insurance plans. We'd be happy to verify the details of your benefits so there are no surprises at your first appointment!

Direct Access

You DO NOT Need a Prescription

New Jersey, and many other states, allow you to seek Physical Therapy without the need of a prescription. This is called Direct Access. This means, you do not have to see another doctor in order to see your DPT!

Cancellation Policy

Our goal is to provide quality health care to all our patients in a timely manner. No-shows, late arrivals, and cancellations inconvenience not only our providers, but our other patients as well. Please be aware of our policy regarding missed appointments.

Appointment Cancellation

When you book your appointment, you are holding a space on our calendar that is no longer available to our other patients. In order to be respectful of your fellow patients, please call us as soon as you know you will not be able to make your appointment. We will try our best to reschedule you versus cancelling your appointment.

If cancellation is necessary, we require that you call at least 24-hours in advance. Appointments are in high demand, and your advanced notice will allow another patient access to that appointment time.

How to Cancel Your Appointment 

If you need to cancel your appointment, please call us at 862-260-9656 between the hours of 8:00am and 8:00pm. If necessary, you may leave a detailed voicemail message. We will return your call as soon as possible.

Late Cancellations/No-Shows

A cancellation is considered late when the appointment is cancelled less than 24-hours before the appointed time. A no-show is when a patient misses an appointment without cancelling. In either case, we will charge the patient a $25.00 missed appointment fee.

Running Late to Your Appointment

In the event that you are running late to your appointment, please notify us. Depending on the time of tardiness, the Doctor of Physical Therapy will decide if rescheduling is necessary. This is to ensure that you can receive the proper amount of time for you care and to make sure that the doctor is not late for their next appointment.
